Home health services are typically provided following a hospitalization or recent subacute rehabilitation service for individuals who are “home-bound.” These services often include the assistance of nurses, physical therapists, occupational therapists, and speech therapists. Home health care services are usually intermittent, lasting about 60 days or less.

On the other hand, Home-Based Primary Care (HBPC) provides continuous care for patients dealing with chronic diseases for as long as necessary, which in many cases can span their entire adult lifespan. Unlike home health services, HBPC does not require a patient to have a “home-bound” status. There may be instances where home health agencies request an HBPC provider to manage “home-bound” patients during transitional phases after hospital/rehab discharge. This is done to stabilize patients and decrease rehospitalizations until the patient can be followed up by their primary care provider (PCP). As noted in a study by Jencks et al., rehospitalization is a frequent, potentially life-threatening event often associated with gaps in follow-up care. HBPC can help to bridge these gaps, reduce inequalities in healthcare access for vulnerable populations, and improve patient care and safety.

PCPs also specialize in advance care planning, chronic care management, and handling acute care issues that may arise. In most cases, lab work and diagnostic tests can be conducted in the home through partnering companies.

HBPC also offers significant palliative care services and frequently collaborates with palliative and hospice providers.

Medicare and most other commercial insurances cover HBPC in the same way they cover traditional in-clinic primary care visits.
